About	Carol Polak (nee Fife), age 72, of Merrillville, passed away October 7, 2009, after a long illness. Carol (January 29, 1937 to October 7, 2009) was preceded in death by her parents: Elbert and Florence Fife; and several brothers and sisters. She is survived by her daughter, Kimberly Polak of Merrillville; son, Donald (Julie) Polak of DeMotte; grandchildren: Brian, Amber and Megan; brothers: Russell Fife of Florida and Jerry (Sharon) Fife of Texas; niece: Cathy Kustric of Hebron; great-great niece, Carissa; and many other nieces, nephews and caring friends.
