About	Elizabeth "Betty" Polomchak, age 95 of LOFS, formerly of Gary, passed away Tuesday, June 26, 2018. Survived by daughter, Carol Polomchak; son, Dale (Carolyn) Polomchak; two grandchildren: Mike (Michelle) Rees, Kristie (Zach) Pyles; four great grandsons: Cole, Brody, and Gavin Rees, and Lincoln Pyles. Preceded in death by husband, Peter; sisters: Mary Szikora, Dorothy Kupka; brothers: Samuel Brozak, Joseph Brozak, and Edward Brozak. Betty was a life-long member of St. Michael Byzantine Catholic Church. She was a graduate of Horace Mann High School. Betty was a retired Claims Deputy for the State of Indiana Unemployment. She loved traveling and was a member of St. Anthony Medical Center Auxiliary.
