59, of Baldwin, PA, died Wed., Nov. 23, 2011. Born Mar. 20, 1952 in McKeesport, a son of Irene "Polly" Paula Zajdzinski Formo of North Huntingdon and the late C. Roger Formo. A Construction Worker for A&D Remodeling, he was a member of Corpus Christi Parish in McKeesport. In addition to his mother, he is survived by his wife, Debra M. Powell Formo; daughter, Ashley Lynn Formo of Pittsburgh; stepdaughter, Crystal G. Winterhalter of Pittsburgh; stepson, James D. Winters of North Huntingdon; sister, Carol Anne (Robert T., Sr.) Masters of North Huntingdon; five granddaughters; half brother, Michael R. Formo of Fergus Falls, MN; a niece and nephews. Friends will be received on Thurs. From 2 to 9 p.m. At the JAYCOX-JAWORSKI FUNERAL HOME, INC., 2703 O'Neil Blvd., McKeesport, PA 15132. Mass of Christian Burial Friday at 11:00 a.m. In Corpus Christi Church. Interment in St. Mary's Polish Cemetery, White Oak.
