Current responsibilities include assessment of home bound patient’s who have been discharged home from the hospital and need further nursing care. I monitor vital signs, perform venipuncture and collect specimens. I instruct on medications, diagnosis, pre-fill medication boxes, administer injections and breathing treatments. I change indwelling foley and supra-pubic catheters and perform wound care on many types of wounds. I have assessed and managed gastrostomy and urostomy tubes. Patient’s are taught how to perform glucometer checks, blood pressure monitoring and diabetic management. This is a small sample of my current duties. Information collected is sent via home health nursing note computer program called Kinnser.
