About	HAYWARD, Jason Lee 45, passed away peacefully September 9, 2018 surrounded by his loving family ( of liver and kidney failure). He was born December 12, 1972 in Grand Rapids Michigan, moving to Florida in 1975. Jason is survived by his mother, Sandra Seib; stepfather, David Seib; sister, Jill Hayward; and three children, Alysa, Kyle, and Austin, all of whom loved him dearly and will miss him very much.
